As you are giving us your personal details such as your name and home address it is important that we
look after that information and for us to explain how we use it.
We want you to feel that Jersey Mencap will keep your personal details safe and respect your wishes.

Below is an explanation of why we have asked you for personal details and how we store it:

 
Why we need your…..
Name and address: so we can send you the list of activities and updates on our projects. We might also
write to you for your feedback and suggestions or to send information that we feel might be relevant or
important for you to read.

Telephone numbers: so we can call you with regard to your chosen activity. If an activity changes or is
cancelled then we need your telephone numbers so we can let you know.

Your date of birth: so we can identify you on our records and also ensure we offer a range of activities
that might be popular with certain age groups. We also send birthday cards to our members.

 
Emergency contact numbers: this could be your parents or carers telephone number. Someone that you
would want us to contact in an emergency such as if you become unwell during a Jersey Mencap activity.
Likes & Dislikes: this is your club so any suggestions you have then please do let us know so we can
keep the activities enjoyable.

 
What do we do with the information? Jersey Mencap will keep your personal information safely stored at
our office. At each activity, staff or volunteers will be given details of who to expect, their contact numbers,
who to contact in an emergency. All our staff and volunteers are trained on keeping this information safe
and private.

By completing this form you are giving us permission to store your personal details. Should you contact
details change as in you move house or change your number then please do let us know.
Jersey Mencap will not pass on any of your personal information to anyone else or use it for anything else
other than the Jersey Mencap projects.
Please note that we regularly post pictures of the social club activities on our website and social media –
please let us know on the membership form should you not want to have your photo taken.

 
What to do if you DO NOT want Jersey Mencap to contact you anymore? To be removed from the
database please inform Jersey Mencap. This is known as your right to withdraw permission. Our
